At Developmental Disabilities Services, we’re dedicated to providing the best care and support for our community. That’s why we have a Quality Assurance Team—a group of experts who work behind the scenes to monitor service delivery.

What does this team do? They make sure everything runs smoothly. Our dedicated team is here to guide Employers of Record (EORs), case managers, and service providers, making sure everyone is on the same page and in compliance with policies. They visit EORs in the recipient’s home to share helpful tips and make sure everyone is up-to-date with the latest guidelines. This way, we can all avoid any citations during audits.

These visits are called Quality Assurance Surveys, and they’re a big part of how we ensure that all records related to your services are being maintained.

Remember, we’re here to support you every step of the way. It’s all about working together to provide the best services possible!

Your Handy Home Record Guide 

Each Employer of Record (EOR) has their own Home Record. It’s a place where you can organize and keep all of the important information that outlines your SDS journey, from a copy of your Plan of Care to Quarterly Reports.

When our case managers or Quality Assurance (QA) Team come by for a visit, they’ll want to review this Home Record. It’s a way for us to ensure everything is on track, and you’re getting the support you need.

Worried about setting it up? Don’t be! We’ve put together an updated guide to make creating and maintaining your Home Record more seamless.

And just a little note on keeping records: It’s best to keep documentation in your Home Record for three months, then store the documents in your home for six years for safe record keeping. It’s all part of making sure we can look back and see all the progress we’ve made together.

Ensuring Excellence: A Conversation with Rhonda Crummett 

In our continuous effort to provide families with insights into the work that ensures the highest standards of service, we conducted an interview with the Program Manager III of the DDS Quality Assurance Team, Rhonda Crummett. Thank you, Rhonda, for all the amazing work you do for DDS! Check out the helpful information that she has shared below regarding QA. 

1. Can you talk about why you conduct home visits?

The objective of Quality Assurance home visits is threefold:  

  • Ensure the individual is getting the services they require.   
  • To verify that EORs have completed training for the individuals.   
  • To confirm that case managers and other DDS personnel are fulfilling their roles in providing comprehensive services and support to the EOR. 

2. What types of trainings need to be included in the Home Record?

All required trainings must be in the Home Record, including the First Aid and CPR training, Foundations, Effective Teaching and Learning, and residential module training. If you are on the In-Home Supports Waiver and have made the decision to exempt your SD-HTS staff from Foundations, Effective Teaching and Learning, and residential module training, you will need to save a copy of the signed DDS-37 form saved in the Home Record.

3. Does the HTS staff need to be MAT certified? What if the HTS staff is the parent?

It is required for HTS staff administering medications during their working hours to obtain MAT certification, even if they are a parent.

4. What do you do when the EOR reports issues or concerns?

Upon receiving reports of issues or concerns from the EOR, the completed SDS form is forwarded to the program administrator of SDS services for appropriate follow-up. If necessary, any field staff may become engaged to resolve case management issues.

5. Do you walk through the home to check the property, refrigerator, cabinets, etc.?

Our protocol does not include an assessment of the home environment, such as inspecting the property, refrigerator, or cabinets, as this is not considered a residential service.

6. Do you need to see financial information?

No, there is no requirement to review financial information.

7. How long does a visit typically last?

Visits typically range from 30 minutes to an hour, depending on the EOR’s inquiries and any concerns they may present.

8. How often do you conduct visits?

The Quality Assurance Team conducts 100 randomized visits annually between August and March of the fiscal year as part of the OKDHS program.

You asked, we answered! Here you will find responses to your most commonly asked questions and helpful information to streamline the EOR process. 

1. How long is the EOR required to keep documentation in the Home Record? 

The Home Record should contain three months of documentation, while the last six years of documentation from the Home Record should be stored in the home in case of auditing.

2. How often and when do I need to send in Quarterly Reports? 

Quarterly Reports should be sent in by the 10th day of the month following that quarter. Please see the example below for a timeline:

3. What things do I report on the Quarterly Report?

The EOR will only report on services that were self-directed. You will report if the specific service was received during this quarter. You will also provide any progress updates for outcomes that are on the plan. The EOR does not report for traditional services such as Occupational Therapy, Physical Therapy, Speech Language Pathology, Nutrition, Nursing, or Behavior Supports. That provider submits their own Quarterly Report. The EOR will also report any changes to the back-up plan and will answer the rep-payee questions if the EOR serves as the rep-payee.
